About the Opportunity Reporting to the Network Manager of Primary Health Care the Primary Health Care Coordinator will provide support to advance the strategic priorities in the Primary Health Care, Chronic Disease Management and Wellness and Department of Family Practice portfolio. The PHC Coordinator will have responsibility for coordination of the planning, ongoing development and implementation of programs, services and initiatives. The incumbent works closely with Primary.
