Community Connectors work with residents and assist them to resolve difficulties affecting their health and wellbeing. You will work both in the community and in a GP surgery, assisting people who have presented at the surgery with non-clinical problems or who have societal problems adversely affecting their clinical condition. You will lead and line manage a team of approximately seven community connectors, supporting them to deliver a first-class service to residents. What you could be part of. As the NHS, Local Authorities and the Voluntary Sector increase their capacity for joint working to meet the multiple needs in the community, this post represents an opportunity to be an active partner in Integrated Neighbourhood Teams increasing population wellbeing through addressing the wider determinants of health. Full training and mentoring will be provided within an established and highly supportive team. You will be availed of regular practice supervision and have ready access to your colleagues and line manager for advice and support. What you'll be doing. backssing and allocating referrals from a variety of stakeholders Providing effective practice supervision to your team Undertaking all first line management responsibilities for a team of Connectors Working with other Lead Connectors and the Specialist in Community Support to ensure that the service operates with optimum efficiency Liaising with GP Practice Managers to ensure that the service meets their needs Representing the service in a variety of multi-agency forums Interviewing residents, backssing needs and co-producing solutions Supporting residents in the medium term to access the help and support they need Liaising with a variety of statutory and voluntary.
