Position Overview The Palliative Care Coordinator functions as a key member of an interdisciplinary team within the Palliative Care Program. The Palliative Care Coordinator in consultation with other team members will have primary responsibility for determining eligibility of clients for the WRHA Palliative Care Program and its components based on established criteria. The coordinator will be responsible for ongoing case management of palliative care clients, including Home Care services. The coordinator will be the primary contact for palliative care team members regarding changes in client conditions and service needs. Experience Two years directly related community experience in Palliative Care required. Community experience an asset. Education (Degree/Diploma/Certificate) A Bachelor’s Degree in Nursing or an acceptable equivalent combination of education and experience will be considered. Certification/Licensure/Registration Current active CRNM Registration. CNA certification in Hospice Palliative Care Nursing an asset. Valid Driver’s License and use of a vehicle. Qualifications and Skills Familiar with philosophy and vision of the WRHA Palliative Care Program. Knowledge of computer systems, word processing and email applications...
