About the Opportunity Reporting to the Primary Health Care Manager, the Primary Health Care (PHC) Coordinator will provide support to advance the strategic priorities in the Primary Health Care, Chronic Disease Management and Wellness and Department of Family Practice portfolio. The PHC Coordinator role will be focused on supporting, reporting, optimization, efficiency and have responsibility to lead the planning, ongoing development, implementation and evaluation of services and project initiatives.
