Associate Care Coordinator required for seniors' living facility If you are a Registered Nurse or a Licensed Practical Nurse looking for a position in a dependable field with one of the most dynamic employers in Southern Alberta, keep reading. Green Acres Foundation is looking for someone with at least 3 years' experience in nursing who has a flexible, willing, and positive attitude. The right applicant will have successfully completed an approved nursing program and have proof of an active registration with CARNA or CLPNA. A current satisfactory criminal record check, a Class 5 driver's license and a clean driver's abstract is required. Related nursing experience in geriatric care is an asset. Basic Responsibilities of the position include: Provide leadership regarding professional nursing practice standards and resident/family-centered care in a lodge setting. Ensure positive resident outcome and satisfaction through clinical training of care staff. Orientate new care staff on Foundation philosophy of care, policies, procedures, and programs. Create an environment that maximizes residents' dignity, independence, privacy, freedom of choice, and decision-making in a home-like environment. Direct, assign, and supervise HCA and LPN employees to ensure proper delivery of services as outlined in resident care plans. Assume responsibility for resident care by identifying service gaps and providing hands-on care if required. Work closely and cooperatively with Management and Care Coordinators. Ensure accurate and complete documentation of care plan, care conferences, charting and other resident information is being recorded in electronic health records or resident files. Ensure compliance with Continuing Care Health Service Standards. Perform other duties as assigned.
