We are searching for a diverse, energetic, self starting Case Manager to join our talented and growing team.Description The Case Manager II for Adult Mental Health Services, provides case management and clinical services for individuals with mental illness, substance disorders use and/or developmental disabilities and their families. Services include supportive counseling, individual and family support, crisis intervention, assessment, Individualized Service Plan (ISP) development, psycho-education, and referrals to clients and their families. Develops and maintains working relationships with: individuals served, families, additional service providers, and other community agencies as needed to ensure quality services. Works as part of a treatment team to ensure services and resources are coordinated among providers as needed to help individuals served live and function optimally in the community. Case Manager II-Bachelor's Degree RequiredStarting Salary: $35,372--Depending on ExperienceCase Manager III-Bachelor's Degree and QMHP RequiredStarting Salary: $37,140--Depending on ExperienceResponsibility * Maintains a minimum caseload of Medicaid funded mental health case management cases as determined by program manager. * Develops effective, behaviorally specific, and measurable ISPs for assigned clients as required by policy. Ensures that ISPs are reviewed at least quarterly and updated as needed and per policy. * Ensures that client records meet licensure and reimbursement guidelines. Accurately completes administrative and other paperwork as required. Through quality assurance methods, ensures all records will be in compliance with applicable local, state, and federal requirements as they pertain to DBHDS licensure, HIPPA and 42 CFR Part 2 standards and regulations, Medicaid regulations, NRVCS policies and any other applicable regulations and/or guidelines. * Promotes positive communication and collaboration among clients, families and/or caregivers, team members, and other community resources to provide quality services. Conducts self in a professional manner at all times. Participates in team meetings and weekly licensure and administrative supervision, unless absence is excused, completes proper written communication and relays other helpful information as necessary by voice mail and/or e-mail. Serves on designated teams, committees and work groups. * Provides direct and related services needed to ensure clients' needs are addressed. Assists with obtaining additional services as needed. Provides psycho-education to client and families as needed. Supports are offered in multiple settings including the community, home, and school. * Provides for follow through and continuity of care including: discharge, referral, placement, follow-up appointments and liaison services. Provides safe transportation in accordance to NRVCS policies and guidelines to individuals served as needed to implement the ISP. * Meets routinely with supervisor to review status of treatment and address any issues significant to the successful treatment of clients assigned. Ensures assigned individuals' assessments are completed prior to program enrollment and in accordance with applicable regulations and NRVCS standards. * Performs at the expected levels of individual and team productivity. Ensures all client records are in compliance with all applicable local, state, and federal requirements including guidelines as they pertain to DBHDS Licensure Regulations, Waiver, State Plan and Clinic Options Medicaid. * Assists with crisis resolution and general problem-solving. Ensures services are provided with respect to the dignity of the individuals served as outlined in the State and NRVCS Human Rights Plans. Advocates for services on behalf of individuals served and their families/care givers as needed. Documents and reports suspected violations of Human Rights according to NRVCS policies. Is accessible from home, occasionally, to help manage client emergencies and crises as if needed. * Ensures all required certifications and/or licensure are kept up to date. Attends/provides other training as required to meet personal professional development and/or program needs. * Completes other duties as assigned by Supervisor, Program Manager and/or Director. Qualifications * Requires demonstrated knowledge of case management/social work practices, and resources available to help children and adolescents with serious mental illnesses, substance use disorders and individuals with co-existing disorders. * Working knowledge of DSM-V, etiology, symptoms, treatments, prognoses. Skills in assessment and service plan development is helpful. * Knowledge and understanding of best practice treatment modalities for serious mental illness, intellectual and developmental, substance use and co-occurring disorders to include behavioral supports and interventions. * Basic knowledge of human rights, ethical, and legal concerns as they relate to providing case management clinical services. * Working knowledge of Medicaid guidelines as they pertain to State Plan, Waiver, and Clinic Option Medicaid. * Must be able to demonstrate problem-solving, critical-thinking ability. Should be able to recognize early escalation of symptoms and/or situations and respond appropriately with de-escalation techniques. * Must be able to exhibit initiative, resourcefulness, and the ability to work independently and as part of a team. * Demonstrated ability to communicate clearly and concisely (written and oral), working knowledge of computers/word processing. * Must possess a valid driver's license (state of Virginia, unless otherwise approved by agency) with an acceptable driving record. Must maintain acceptable record throughout employment. * Must have an acceptable criminal history background and child protective services check upon hire and maintained throughout employment; * Must have acceptable drug screen results upon hire. Education/Experience Required: * Bachelor's Degree is required in psychology, counseling, social work or special education, but will also consider an unrelated degree. * Experience in a setting working with individuals with mental illness, substance use disorders and/or developmental disabilities is preferred Additional compensation will be considered for any new employees who speak multiple languages.
