Comfort Home Care is in search of Weekend Per Diem Visiting Registered Nurses in MSNE; Acton, Concord, Littleton, Maynard, Fitchburg, Leominster and the surrounding areas. Duties and responsibilities: Comfort Home Care is seeking a Massachusetts licensed RN/Registered Nurse for visiting patients in the home that can: Provides initial and ongoing nursing assessments to identify the client's medical needs, level of functioning and ability to communicate and learn. Initiates the plan of care, necessary revisions, and performs OASIS assessments at appropriate time points when applicable. Initiates and performs appropriate nursing interventions. Coordinates services, informs the physician and other personnel of changes in the client's condition and needs. As an RN, collaborates with patient /family and other health care providers and/or community resources with planning of care and discharge.
