JFSWNY has an exciting opportunity for a an Adult Health Home Care Coordinator who is Spanish Speaking. Do you have a passion for working with our Spanish-speaking clients to help ensure that they receive comprehensive care coordination? Do you have the ability to advocate on the behalf of clients to resolve problems that may restrict their access to needed medical services? Do you have a case management background and the demonstrated ability to work effectively and sensitively with refugees and immigrants? If so, then this full-time position provides a unique and rewarding opportunity for you to make a difference in the lives of many. WHAT YOU'LL DO In working with your clients, you may be needed to coordinate interpretation and provide translation. Other times, you'll arrange provider appointments and providing transportation there when necessary. In all cases, you'll be promoting client rights and access to social service, social security, mental health, and other support services. Equally important, however, are keeping accurate documentation , maintaining case notes, and completing/monitoring all required forms. For this reason, you must not only have proficient communication skills but must also have excellent writing skills and attention to detail. . WHAT YOU'LL BRING Minimum of a Bachelor's degree in a human services related discipline preferred Fluency in speaking Spanish desired 2 or more years' experience with community based client services or equivalent Ability to effectively provide case management services, responding supportively, and with persistence, to client families Demonstrate an understanding of the community and resources Familiarity with community linkage resources helpful Willingness to work within the community, complete home visits and occasionally transport clients Data management for reporting and documentation Experience with electronic medical records a plus Proficient verbal & written communication skills for interactions and for documentation Familiarity with and appreciation of issues related to people with diverse cultures and languages WHY JOIN JFS? The starting wage for this position is between $20 - $22.50 (midrange) per hour dependent on experience and language skills and offers great benefits including 4+ weeks accrued Paid Time Off (PTO), plus 13 paid holidays, excellent health insurance options (IHA), 401k w/match and much more. We also have a reduced 35 hour workweek with an early close on Fridays! We also strive to maintain small caseloads to help prevent employee burnout and to ensure that we can provide quality care to our clients. Hybrid schedule options available and flexible scheduling. We are conveniently located at the Tri-Main Building at 2495 Main Street. Since this position requires work in the community, you must have a reliable vehicle, a clean driver's license and required liability insurance levels of 100/300K.
