Where your work is valued, you are in a safe and secure place, and most of the patients are very appreciative of your care? Position Description: Conduct rounds with physicians to meet with patients to discuss placement orders. Assist patients with a specific plan for medication management. Include patient in discharge planning. Assist patients with the identification of a support network, instructions for future care and follow-up. Coordinate care with community agencies, shelters, employment agencies, and drug and alcohol counselors.
