Duties and Responsibilities Lead communication and coordination with school personnel, families, and patients to ensure adolescent patients continue to receive a quality, individualized education while engaged in treatment. Collaborate with the multidisciplinary team to implement a plan for patient education and any needed educational modifications and supports as identified by IEPs and 504s. Oversee daily education time during program to ensure a safe, supportive, and structured learning environment for patients. Facilitate patient access to community resources, including housing, food, clothing, educational supports, vocational opportunities or services, life skills, mental health services, needed medical support and social/community supports. Facilitate discharge planning to ensure continuity of care and ongoing support with providers post discharge from EDCare. Document all patient encounters appropriately. Perform miscellaneous job-related duties as assigned. Strong interpersonal and communication skills with the ability to build rapport quickly, facilitating positive relationships across a wide range of constituencies, including patients, families, multidisciplinary teammates, and school-based personnel. Works effectively with a wide range of individuals and systems in a diverse community. Participates in IEPs and 504s. Monitor, assess and record client progress and adjust plans and services as needed in collaboration with other team members. Knowledgeable of community health care, educational supports, vocational services, and support services. Communicates pertinent clinical information to outside providers or other individuals as helpful for coordination of care and resource development. Utilize an EMR to document patient interactions. Minimum Qualifications: Bachelor's degree required. 1 year of experience directly related to the duties and responsibilities specified required. Completed degree(s) from an accredited institution that are above the minimum education requirement may be substituted for experience. Computer Skills: Minimum of beginner skills in Microsoft Word, Excel, and PowerPoint. Intermediate skills are required with Microsoft Outlook. Must be able to effectively maneuver within an office scheduling system. Must be able to efficiently learn and effectively work within an electronic medical records system. Physical Demands and Environment: Physical demands of this job include but are NOT limited to: Sitting frequently, standing occasionally, walking occasionally, talking frequently, hearing frequently, seeing constantly, reaching with arms and hands occasionally, crouching occasionally and using fingers intricately. This position requires lifting/carrying up to 10 pounds occasionally. Environmental conditions of this job include but are NOT limited to: 95% inside, 99% normal temperature and 99% normal noise as expected in a medical office environment.
