Hospital Liaison

Full-time 35 hours per week NOT Remote Bilingual English/Spanish fluency is preferred but not required. The position will be co-located at Baystate Medical Center and GSSSI. This position is currently grant-funded for two years but may continue after the initial pilot period. Additionally, other internal opportunities can be explored for the right candidate. Full primary series of COVID-19 vaccination is required (at least first shot before employment begins) with one booster shot required when eligible. The Hospital Liaison (HL) is a two-year grant-funded position. As an employee of GSSSI, the Hospital Liaison will work as part of an interdisciplinary team that includes hospital case managers, social workers, and ancillary staff. The Hospital Liaison will provide consultation, support case reviews, and person-centered recommendations for home and community-based service and supports for individuals over sixty (60) to return home when possible. The Hospital Liaison will provide in-person, telephonic, or virtual consultations to individuals seeking to explore hospital discharge to home rather than a skilled nursing facility or other institutional settings. In addition, the Hospital Liaison will provide limited short-term follow-up on referrals or information provided. The Hospital Liaison role has ongoing training requirements, including conducting periodic community resources training and materials for hospital staff. Bachelor's degree preferred or an Associate's degree with 2-3 years of relevant experience Requires experience and working knowledge of public benefits and community resources. Public speaking experience is strongly recommended Experience with older adult service network preferred. Education or relevant experience with Alzheimer's disease and related dementias is desirable. Public Service Loan Forgiveness (PSLF) If you are employed by a not-for-profit organization such as GSSSI, you may be eligible for the PSLF Program for student loans. Job Posted by ApplicantPro
