About Us Care Lync Georgia is a Social Services agency focused on providing Service Coordination (Supports Coordination/Case Management) services to adults who can no longer live independently or suffer from a Physical Disability throughout Georgia who are enrolled in the Department of Human Services, Medicaid Waiver Program. Our Service Coordinators work with adults to help them live independently in their home and community. General Summary Provides case management activities necessary to meet the needs of clients assigned to the consolidated care team. This position is responsible for the development of individualized care management plans; implementation of the care plan through brokering and coordinating services; and the monitoring and evaluation of all clinical outcomes to ensure that services are provided in a professional, comprehensive, and cost-effective manner. Essential Duties and Responsibilities Development of Care Plan. In-Home/In Person Assessment Communication of Care Plan and HCBS needs to all members of care team. Annual case review and "as requested" by the administration to determine time, scope, amount, duration and frequency of services based on consumer needs. Enhance collaboration between the company and health care providers to promote teamwork and communication among entire healthcare team. Other duties as assigned. Educate, mentor, support, and monitor the eligible participants choosing to direct their Personal Support Services (PSS) Assess needs, develop, implement, and review care plans Make appropriate community service referrals Coordinate all services, Medicaid and non-Medicaid Monitor service delivery and client health and safety Review case notes, EDWP Notification forms, and the Comprehensive Care Plan to assess the clients' ability and capacity to make decisions regarding his/her care needs and services Monitor all care activities, health and welfare, support and socialization, and client satisfaction Maintain monthly contacts with the client/representative to monitor the clients' care and provide mentoring and support to the client in his/her role Qualifications 1 Year Required Social Services or Human Services Two years of experience in case management in a health care field. Experience with the CCSP and Source Waiver programs is a plus. Reliable transportation Meet with clients, Nurses, and other key staff as needed to develop and coordinate treatment plans Conduct monthly contacts and in person-assessments and visits as detailed by the CCSP and SOURCE requirements Must be able to function independently and effectively in a community-based environment. Proven interpersonal skills with ability to communicate effectively. Proven decision-making skills, organizational, self-disciplined, and time management skills. Must be familiar with general use and functions of the computer, such as, usernames and password concepts; internet; e-mail; navigation of computer desktop or laptop, including starting programs, using files, and windows, effectively use navigation buttons and tool bars.
