We offer a COMPETITIVE WAGE AND BENEFITS package DOE. Previous experience working in long-term care is REQUIRED. Responsibilities: The Social Services Assistant assists the Social Services Director in providing services to meet the social and emotional needs of the residents/patients. Assists with POLST and Guardianship assistance (prior experience and knowledge in this area is REQUIRED) Develops a rapport with the residents/patients and presents their concerns to center management. Charts as assigned Assists with Resident Council Assists the Social Services Director with discharge planning for residents Follows all center's policies and procedures Qualifications: High school diploma or equivalent Ability to speak, understand, write and read in English Previous experience in a long term care setting REQUIRED Don't wait or hesitate and forward your resume today!
