The PACE model is centered on the belief that it is better for the well-being of seniors with chronic care needs and their families to be served in their community whenever possible. Seniors, their family members, and caregivers face many issues, including transportation to appointments, management of medications, coordination of medical care from different specialists, lack of social interaction, and ability to stay alone at home. On Lok PACE participants receive in home care services and transportation to a On Lok PACE center for primary medical care, social and recreational activities, and other senior care services. POSITION SUMMARY: Under the administrative supervision of the Program/Center Manager, the clinical direction of a Center RN, and the clinical supervision of the associate nurse manager, the Licensed Vocational Nurse provides direct nursing services to On Lok participants. DUTIES / RESPONSIBILITIES: Processes, communicates and implements all medication orders. Administers and distributes all medications as prescribed by On Lok medical staff. Provides nursing care and treatments for participants at the Day Health Center. Maintain clinic supplies. Participates actively in the interdisciplinary team by offering clinical expertise in team planning, problem solving, and processes. Teaches, supervises, and counsels the participant, geriatric aides, and family members regarding nursing care needs and other related problems of the participant. Other related duties as assigned. QUALIFICATIONS (knowledge, skills, abilities): Graduate of an accredited LVN program. Possess a current California LVN license. Interest, demonstrated ability and one-year experience working with the elderly or frail population required. Ability to work independently and as part of multidisciplinary team. Bilingual ability may be required for specific assignments. The above statements are intended to describe the general nature of work performed. Sign-On Bonus Plan You are eligible for a sign-on bonus in the amount of $2,000. The first payout is contingent upon completing a successful onboarding "probationary" performance period after 6 months of employment. The second payout is at 12 months and the final payout at 24 months. You must maintain employment at On Lok for a minimum of 2 years from the date of hire to receive the final payment. To be eligible, you must be actively employed and in good standing by the time of each payout in order to receive the payment.
