POSITION SUMMARY The Intensive Care Coordinator is part of the Care Coordination and Support program and provides care coordination to youth who would benefit from the coordination of community services. This program is created in response to the creation of HFS Pathways to Success. The roles of Care Coordination and Support are to partner with families to understand their vision, help create Crisis Prevention and Safety Plans, help the family build a Child and Family Team, and provide education and linkage to resources and services that will help the family and youth achieve their goals. The Intensive Care Coordinator is responsible for designing and delivering services that meet the youth's needs and providing support to youth and families who have been referred to the Intensive Care Coordination Tier of Pathways to Success. ESSENTIAL FUNCTIONS AND RESPONSIBILITIES Evaluate and interpret the referral information and attempt contact with the family within seven days of referral. Complete a strength needs and cultural diversity assessment with the child and family and use the IM+CANS to create an individualized strengths-based service plan. Responsible for reviewing initial IM+CANS and then updating any subsequent IM+CANS as required by HFS. Uploading documented into the HFS IM+CANS Provider Portal. Develop, document, and maintain a Crisis Prevention and Crisis Safety plan with the youth, parent, guardian, or caregiver. Responsible for assisting families in identifying natural and professional supports to create a Child and Family Team and facilitating CFTMs every 60 days and 48 hours after a crisis event. Engage family weekly with at least one monthly face-to-face home/community visit. The coordinator will provide linkage to in-network providers of any recommended services; will collaborate with the youth's MCO Case Manager, as applicable. Deliver strength-based, family-centered, culturally competent, and trauma-informed facilitation services. Engage with treatment providers, schools, and other collateral contacts to meet youth's needs. Maintains case records and notes in a timely and professional manner; per the duties of the Care Coordination and Support Organization. Maintain a caseload of 1:25 within the Designated Service Area Completes other duties as assigned. SKILLS and ABILITIES Strong organizational and time management skills. Excellent interpersonal and written communication skills. Must possess the knowledge, personal qualities, and practice skills to work effectively with professionals in other organizations and from other disciplines and to provide services that reflect the values of the System of Care. EDUCATION AND/OR EXPERIENCE Bachelor's Degree in counseling and guidance, rehabilitation counseling, social work, education, psychology, or related human service field, along with two years of supervised experience. The ability and willingness to actively participate in a fast-paced, multidisciplinary team environment. Knowledge of community programs, services, evidence-based practices, and various treatment modalities used to work with youth and their families to navigate child-serving systems. Must possess and maintain a valid driver's license Must possess and maintain proof of auto insurance Must be at least 21 years of age CARE & TCI One of the essential job functions of any position within the agency is to align your performance with our overarching philosophy of CARE. All decisions, actions, communication, and performance should align with the six CARE principles (Developmentally Focused, Family Involved, Relationship Based, Ecologically Oriented, Competence Centered, and Trauma Informed). Not only must you complete the CARE training with satisfactory participation, but you must then put that training into practice in your everyday work. Every employee must also pass TCI training with a satisfactory grade in written and/or physical testing. All employees must remain current on refresher training that is provided monthly at all locations. You will be tested every six months on the physical portion, if applicable to your position. You will be tested annually on the written portion, regardless of your position. If you are not current on your TCI refresher training every six months, you will be required to retake the full course. If you do not pass either test, you may be required to retake the full course.
