Millennium Physician Group is seeking to hire a full-time / Case Manager to join the ACO team in the Naples, FL area . We are looking for a new team member who is positive, energetic, well-organized, has the ability to multi-task, and think outside of the box. We want someone who supports our top initiative of ensuring an excellent patient experience! The Case Manager is responsible for the direct provision of services that assist patients with the achievement of their highest level of wellness. S/he is focused on client centered services that link clients and other family members with health care, psychosocial and other home and community-based services to ensure timely, coordinated access to medically appropriate levels of health and support services and continuity of care. S/he is responsible for assessment, planning, coordination, advocacy, education, monitoring and accurate, timely documentation in all program areas. S/he demonstrates knowledge of growth and development and possesses the ability to assess, interact, communicate and care for age specific patient populations. This position earns competitive compensation plus a full benefits package including 401(k) with match and 3 weeks of PTO! Required Knowledge and Experience Registered Nurse; active and clear license in State of Florida. License must be maintained once employed. 3+ years of clinical experience with at least 1 year of case management experience, preferred. Required Skills and Abilities To perform this job successfully, an individual should have strong computer skills especially in Excel, and knowledge of EMR systems; Athena knowledge preferred but not required.
