***$5,000 SIGN ON BONUS*** VNA of Kansas City Home Health and Hospice is seeking a full-time Registered Nurse to join our award-winning team and provide home health services in the Kansas City metro and surrounding areas. This position provides exceptional autonomy while being connected to a multi-disciplinary team with Managers who have all been Case Managers, and continue to support the team with all needs. Our nurse case managers have enjoyed being part of an organization that is dedicated to maintaining a separate "After Hours" Team, allowing case managers to work Monday through Friday. Benefits include: Medical, dental and vision plans start on the 1st day of the month following the month of employment PTO starts to accumulate from first day of employment In addition to PTO, six paid holidays per calendar year 403(b) with company match Self-determined schedule The Work of the Home Health RN IV Case Manager Includes: Completes an initial, comprehensive and ongoing comprehensive assessment of patient and family to determine needs. Provides a complete physical assessment and history of current and previous illness(es). Administers a variety of IV medications and provides IV care to a variety of access devices according to best practices. Other duties as assigned. Provides professional nursing care by utilizing all elements of nursing process and as defined in the state Nurse Practice Act. Assesses and evaluates patient's status by: Writing and initiating plan of care Regularly re-evaluating patient and family/caregiver needs Participating in revising the plan of care as necessary Initiates the plan of care and makes necessary revisions as patient status and needs change. Uses health assessment data to determine nursing diagnosis. Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process. Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician's plan of care. Counsels the patient and family in meeting nursing and related needs. Provides health care instructions to the patient as appropriate per assessment and plan. Assists the patient with the activities of daily living and facilitates the patient's efforts toward self-sufficiency and optional comfort care. Acts as Case Manager when assigned by Clinical Manager and assumes responsibility to coordinate patient care for assigned caseload. Maintains regular intervals of comprehensive assessment according to and in compliance with state and federal regulations. Applicants must have: A valid Missouri and Kansas RN License to Practice.
