Full Time. Weekend and holiday rotations. The primary responsibility of the Clinical Field Educator is to provide a continued education presence to clinicians from the time they successfully complete clinical orientation until their 2 year anniversary. The goal is to ensure the clinicians have the solid foundation necessary to move from novice to expert as hospice and palliative care clinicians. Essential Duties and Responsibilities for the Field Educator: Coordinate and document progression and competence by making routine and prn visits with all clinicians who have successfully completed orientation through their 2 year anniversary in the following skill sets: Time management Case Management (as applicable) Crafting a recert note Symptom Management assessment and intervention Hands on Skills End of life Care The art of the conversation Managing difficult family dynamics (Level One) In collaboration with the Clinical Education Coordinator and the VP of Clinical Education, the Clinical Field Educators will: Apply the clinical inventions developed for Education Remediation referrals Assist the team managers with field supervisions for annual reviews Assist with record reviews for clinical accuracy and compliance Assist teams during periods of short staffing/increase demands for visits Serve as back up for preceptors Engage in Cross Training when necessary Shifts - will be asked to conduct visits on weekends and off shifts within all clinical departments Non-Essential Duties and Responsibilities: Expected to perform additional duties as requested. Qualifications: Education/Certification: Graduate of an accredited school of nursing with current RN licensure in the State of New York. If applicable, Valid Driver's License, free of serious violations, and vehicle required for use when on duty or on-call. Experience: Minimum of 2 years hospice and/or palliative care experience preferred Physical Demands: Walks and stands for most of the working day. Position requires sitting, standing, bending and reaching along with a normal range of hearing and vision. Must be able to negotiate stairs, walkways, driveways, and other environmental objects while out on appointments, in patient homes, or visiting facilities. Requires manual dexterity sufficient to provide hands-on care to the patient. Must be able to lift and maneuver a minimum of 25 pounds Must be free of communicable diseases. Must be able to operate standard office equipment such as computer, mobile device, phone, copier, scanner, printer, or related items.
