Grand Itasca Clinic & Hospital, a part of Fairview Health Services, is currently seeking a Visit Facilitator to join our team! POSITION: Visit Facilitator Float (Nursing Assistant, HUC, or EMT) EMPLOYMENT TYPE: .5-.7 FTE (40-56 hours in a pay period) WORK SCHEDULE: Days DEPARTMENT: Clinic Internal Posting through 2/8/23 JOB SUMMARY: This position works primarily in family practice primary care and is responsible for supporting in person clinic visits, including communicating with patients prior to their visit and helping guide patients through the care experience. The visit facilitator gathers pertinent medical information from the patient and documents the information into the patient's medical record. This role also helps to ensure the exam rooms are stocked, equipment and supplies are available and assists with tasks that support the effectiveness and efficiency of the team. JOB DUTIES: Prepare patients for their appointments including communicating with patients in advance of their scheduled appointment as needed to ensure medical records are collected, received and available for visit if applicable. Assist in the rooming process at the time of the in person visit by collecting and documenting pertinent information such as the reason for their visit and applicable health questionnaires. Prepare exam rooms and stock supplies. Supports the patient connection to the provider for the visit and serves as a resource ensuring a positive patient/provider experience. Completes visit wrap-up ensuring the after-visit summary is provided and patients are aware of recommendations for follow up. This may include assisting with scheduling and coordinating follow-up appointments. Communicates effectively with patients, providers and other health professionals, demonstrating comfort in person and with virtual technologies. QUALIFICATIONS: Must be at least 18 years of age. Successful completion of Nursing Assistant, Emergency Medical Technician and/or Health Unit Coordinator program required. Current registration or certification preferred. Minimum 1-year of customer service experience required. Previous health care customer service experience preferred. Proficient with technology (computer hardware and programs, smart devices such as phones or tablet). Appropriate verbal communication skills. Strong team player and ability to work independently. Ability to maintain strict confidentiality. Demonstrated ability to follow and adhere to Grand Itasca Clinic and Hospital policies and procedures, vision, mission, and values.
