Position Overview: Provides comprehensive nursing care and rehabilitation nursing staff leadership. Assesses patient needs, plans and implements patient care, and evaluates the patient's response to treatment. Manages care within the therapeutic environment and directs the delivery of nursing care by other members of the nursing treatment team. This position may also be responsible for performing CNA and clerical duties as needed. Essential Functions : a) Uses acceptable standards of practice based on nursing process and theory to help the individual/family/group, with complex and unpredictable needs, identify and meet basic age-appropriate health needs. b) Promotes a climate in which consumers can act in their own interest or intervenes when they are unable to do so. c) Uses the nursing process and theory to provide age-appropriate individual/family/guardian/designated surrogate education using a wide range of teaching activities to promote health. d) Works to achieve desired health care outcomes by effectively utilizing human and material resources. e) Is accountable for the ethical, legal and professional responsibilities related to nursing practice across all the roles, within the scope of practice. f) Performs admission and ongoing assessment related to patient condition and/or plan of care. g) Supports the interdisciplinary team model. h) Provides and delegates rehabilitation nursing intervention based on the interdisciplinary treatment plan and established priorities. i) Develops and maintains effective communication with the patient, family and team members. j) Participates in unit performance improvement activities. k) Promotes the concept and image of professional rehabilitation nursing practice. l) Coordinates discharge planning, instructions, and follow up care. m) This position may also be responsible for performing CNA and clerical duties as needed. n) Performs other duties as assigned. Physical Demands and Working Conditions: Physical Demands: Ability to lift twenty (20) pounds. Moving, lifting or transferring of patients may involve lifting of up to fifty (50) pounds, as well as assist with weights of more than one-hundred (100) pounds. Ability to stand for extended periods. Fine motor skills. Visual acuity. Qualifications & Skills: a) Education: Graduate of a nationally accredited school of nursing. Persons in this position must be a licensed Registered Nurse in Colorado, or a licensed Registered Nurse in a Nurse Compact State. BLS required; ACLS preferred. b) Training & Experience: Academic and clinical experience in rehabilitation nursing (CRRN) desired but not mandatory. c) Job Knowledge: Familiar with post operation patient care and joint replacement rehabilitation.
