JOB SUMMARY This position exists to provide education, interventions, and linkages to resources to support patients in their healthcare goals including disease prevention and chronic disease management. Focus areas include OB and pediatric patients and supporting patient in their family planning needs. ESSENTIAL FUNCTIONS Assist patients with the identification, selection, monitoring and documentation of self-management goals, monitoring the patient's progress toward goal achievement and modifying as needed. Educate patients and families on disease state and symptoms, therapy, and lifestyle choices to promote wellness by planning, coordinating and providing patient-specific education based on the chronic or associated conditions identified. Coordinate care between patients and their internal and external service providers to improve the flow of information between entities to ensure effective treatment plans. Complete psychosocial assessments with patients and serve as an advocate for the patients and recipients of family planning services to understand needs (i.e., shelter; transportation, child care, safety) Provide extensive care coordination and case management of identified youth, families, and OB patients, including management of patient emergency department and hospitalization reports. Work collaboratively with medical providers, nurses, and integrated behavioral health, to prioritize needs of the patients, prepared via pre-visit planning, to provide daily updates to the team in huddles related to care coordination, including external services, resources, and patient education needs. Provide direct patient education related to pregnancy, newborn care, pediatric growth and development, and basic nutrition and exercise throughout preconception to post-partum and pediatrics. Provide brief interventions to educate and support IHB patients to complete preventative screening tests, make informed decisions related to family planning options, immunizations, and adopt healthy lifestyles. Develop and maintain a registry of patients receiving family planning counseling and methods, targeting follow-up of recall for family planning method provision including periodic STI screening as needed. Develop and maintain tracking system of family planning recipients who are contemplating contraceptive options, to encourage initiation. Provide follow-up by appointment or phone to encourage safe and consistent utilization of family planning methods to recipients of family planning services. Work collaboratively with medical provider to develop and maintain tracking system for OB care and post-partum medical appointments. Serve as a linkage between patients and community resources, familiarizing self on insurance coverage (e.g. car seats) of resources or low to no cost resources meeting patient priority needs (e.g. safe sleep options for newborns). Develop and maintain tracking system for pediatric immunizations and asthma plans, working with pediatrician and families to meet the needs of the patient. Create, maintain, and organize current and culturally appropriate health education materials related to position. Track and evaluate program data, completing reports (i.e., monthly board and grant-related reports), as scheduled EDUCATION AND EXPERIENCE Bachelor's degree in Maternal and Child Health, Health Education, Social Work, or Public Health preferred. Three years' experience providing health education, case management, and/or care coordination in the American Indian community preferred. Must demonstrate a passion for OB/newborn care and pediatrics. Ability to show initiative and resourcefulness, working well independently as well as on a team with medical providers and staff. Excellent verbal and written communication skills in a clinical setting, to include EMR documentation. Exceptional computer skills in Word, Excel, and Outlook Proficiency with Electronic Medical Records HIPAA IHB makes reasonable efforts to limit access to and use of protected health information (PHI) by employees to the minimum necessary performance of assigned duties as outlines in job descriptions. This position is access restricted to PHI needed to carry out health care operations. Important: The COVID and Influenza vaccine are required at Indian Health Board
