Job Title: Case Manager Status: Full-time, Mon - Friday (hours may vary) FLSA Classification: Non- Exempt Job Location: Far Rockaway, Queens OBJECTIVE We are seeking an experienced Case Manager for Lantern's Men's Shelter in Far Rockaway, Queens. Case Managers are part of a interdisciplinary team aimed at stabilizing shelter clients by improving their independent living skills and assisting clients in moving towards their housing goals. Case Managers are responsible for assessing client needs, linking client to services, planning a course of action towards stabilization and re-housing, monitoring progress, and documenting client contacts. The successful candidate will have prior experience working with homeless and/or mental health individuals. ESSENTIAL FUNCTIONS Reasonable accommodations may be made to enable individuals with disabilities to perform the essential functions. Accomplishes clients' care by assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress. Admits new clients by reviewing records and applications; conducting orientations. Determines clients' requirements by completing intake interviews; determining the mental and medical needs of the client. Establishes treatment programs by setting schedules and routines; coordinating services being provided; arranging resources, including transportation and escort. Monitors cases by verifying clients' attendance; observing and evaluating treatments and responses; advocating for needed services and entitlements; obtaining additional resources; intervening in crises; providing personal support. Communicates clients' progress by conducting weekly interdisciplinary meetings and evaluations; disseminating results and obstacles to therapeutic team and family; identifying treatment influences. Improves treatment results by studying, evaluating, and re-designing processes; implementing changes; rewriting policies and procedures. REQUIRED QUALIFICATION Bachelor Degree in Social Work or related field Experience working with the homeless, mentally ill, substance abuse and/or low-income populations. Strong relationship building skills required. Strong Computer skills required including Word and Excel. Ability to work Evenings and/or Weekends on a rotating schedule. Preferred experience working in a homeless shelter. Knowledge of mental health, substance use or homeless Bilingual (English/Spanish) preferred.
