***Current $4,000 Sign-On Bonus*** Standard Shift: Day Shift Weekly Schedule: Monday to Friday Rotation Weekends Benefits: Health Insurance (PMCC will pay 80% of the premium) 401(k) 401(k) Matching 403(b) Dental Insurance Vision Insurance Life Insurance Disability Insurance Paid Time Off Loan Forgiveness Tuition Reimbursement Employee Assistance Program Referral Program Duties and Responsibilities: Prepare reports as requested Attend various committee meetings of the facility (i.e., infection prevention and control, policy advisory, pharmaceutical, quality assurance and performance improvement (QAPI etc.) as required Participate in developing the agenda for committee meetings as necessary Participate in regularly scheduled reviews of the discharge plans Job Description: Managing admissions and discharges within a particular department. Supervising and guiding other nurses in their work. Encouraging a culture that promotes staff productivity. Monitoring patients, administering medication and reporting patients' condition to doctors. Maintaining patient records as per requirements. Ensuring that the unit runs efficiently and as per standards and regulations. Provide oversight of Certified Nursing Assistants (CNA's) and licensed nurses as directed by the Director of Nursing. Consult and coordinate with the interdisciplinary team (IDT) and healthcare professionals to assess, plan, implement, and evaluate individualized resident care plans. Ensure initial baseline and periodic comprehensive assessments and care plans are completed within required timeframes. Participate in completing sections of the Minimum Data Set (MDS) assessment as requested. Monitor the chronic health conditions of residents; be familiar with reportable changes and potential causes for concern. Ensure that newly admitted residents have physician orders for immediate care. Provide nursing care that is compassionate and sensitive to residents with cognitive decline, memory loss or history of trauma. Provide support and assistance with resident personal care and activities of daily living (ADL's) as needed. Assist with the admission process; provide resident orientation as appropriate. Monitor the skin health of the resident; provide preventative skin care; administer wound treatments as ordered. Charting and Documentation Functions: Complete and file required recordkeeping forms/charts upon the resident's admission, transfer and /or discharge; provide notifications to the ombudsman and responsible party per requirements. Document reports of accidents/incidents involving residents; follow established procedures. Perform routine charting duties as required and in accordance with established charting and documentation policies and procedures.
