Spanish Speaking Family Support Specialist (Case Manager)

Description:
The Family Support Specialist is a member of Bridges Homeward's Developmental Disabilities Program team, and assists families, children, youth and adult family members referred by the Department of Developmental Services (DDS) in developing life skills, setting and achieving goals, assisting in accessing resources needed to obtain goals, engaging in activities in the community, and working on behavioral and educational issues. HOW YOU WILL BE MAKING A DIFFERENCE: Provide intensive support and services to individuals who have an IEP, live in their home and would be at-risk for placement in a residential setting if not for the in-home services provided; Develop strengths-based partnerships with families to help provide relief and support. Collaborate with families to assess their needs, establish goals and to develop a plan to achieve those goals. Create behavioral plans to help a child build skills and decrease challenging behaviors; Provide parent training and behavioral consultation. Advocate effectively for our client families. Facilitate communication with the team members, including DDS, school, medical and in-home providers and therapists; Conduct one-on-one in-home or community based visits with individuals and/or families as required, based on family need or contract requirements to monitor effectiveness of support plan; Assist families in accessing resources needed identified in the Family Support Plan; Collaborate with other Bridges Homeward staff in developing and coordinating family activities and programs; Assist in recruitment, assessment and evaluation of respite care workers and skilled trainers for identified families as needed; Conduct and prepare assessments of families as needed What Bridges Homeward will do for you: In addition, for an opportunity to join a group of progressive, enthusiastic professionals with a shared passion for human services and child welfare, Bridges Homeward offers benefited employees: Professional Development Health and Dental Insurance 403 (b) Retirement Plan with employer match Generous Paid Time off Life Insurance Long Term Disability and Short Term Disability REQUIRED EDUCATION & EXPERIENCE: Bachelor's degree in social work, psychology, education or a related field; 3 years' experience working with families, children and adults with developmental disabilities may be substituted for the Bachelor's degree. Minimum of 1 year's experience in working with families, children and adults with developmental disabilities. Knowledge of positive behavioral techniques and ABA principles. Demonstrated ability to provide case management to families, including ability to assess needs, establish goals and to develop and implement action plans. Familiarity with special education Excellent written and verbal communication skills Spanish speaking preferred Other requirements: Valid driver's license and properly insured vehicle Clearance of background checks as required by state and federal law.
