Care Delivery Manager

Description:
The position has oversight and management of central nursing services between clinical areas of Primary Care and Specialty Care and is responsible for the development, recommendation, decision support, management, and implementation for the following programs and departments; Centralized Rx Refill, Prior Authorization, Anticoagulation. This position manages program deadlines, certifications, and budgets, including tracking revenue and expenses to ensure program viability. This person supervises staff supporting these programs. Plans and implements key SMG and Primary Care goals and objectives including but not limited to implementation of best practice, quality improvement initiatives and execution of practice teams. Assures implementation of patient-centered goals. This position works closely with the leadership team to provide care delivery support for the clinic. Leads, mentors and coaches staff. Works in collaboration with other primary and specialty care leaders to coordinate care clinic sites to assure the patient care experience is seamless for the patient.This position is expected to support the mission, goals and objectives of the Stillwater Medical Group. Education: Current RN licensure in Minnesota and Wisconsin or upon hire Three (3) years related nursing experience in a clinical setting. Knowledge of nursing theory, regulations and practice Three (3) years of management/supervisory experience preferred. BLS certified Skills required: Advanced organizational skills Excellent verbal and written communication skills in English. Ability to learn and retain information regarding patient care procedures; Ability to plan, prioritize and complete delegated tasks Ability to project a pleasant and professional image Excellent communication skills Experience required: Experience with project management or leadership role and/or project management/operational process design Proven ability to manage multiple priorities daily.
