HOURS - Shifts are Day shifts (9 a. m. to 5 p. m. ) and Afternoon shifts (1 p. m. - 9 p. m. ) per week. Shifts are scheduled Sunday to Thursday. Some flexibility in work hours may be required. Who We Are The work of Good Shepherd is based on a fundamental belief in providing hope and restoring dignity to the most vulnerable members of our community. Our services include emergency food transitional housing and education for homeless and street-involved youth; hospice palliative care; community mental health programs; and supportive housing programs. We are committed to integrating Harm Reduction practices into our programs and to prioritizing the rights, dignity, and choices of those who use substances. By integrating a Harm Reduction framework at Good Shepherd, we can decrease barriers to service and better support some of our community's most marginalized members. This opportunity is with our Good Shepherd - Family Centre How You Will Make an Impact The Rapid Mobile Case Manager will provide shelter diversion supports to families who are at imminent risk of homelessness. The shelter diversion team assists families in identifying safe and appropriate alternatives to shelter accommodation. Short term case management and brief resolution services including financial supports, advocacy, information, referrals, mediation, and support with a housing search are offered Work closely with emergency shelters and partner agencies to leverage and coordinate the assets and services available in Hamilton to ensure that families have their individualized housing and support needs met Have a demonstrated knowledge of various governmental and non-governmental systems and an understanding of how to assist people in successful navigation through these systems An in depth understanding of trauma as it relates to homelessness and housing stability is also necessary Individuals will work from a strengths-based trauma informed model of service and an anti- racist/anti-oppressive.
