Apply for JobJob ID359697 LocationTwin Cities Job FamilyHealth Care Full/Part TimeFull-Time Regular/TemporaryRegular Job Code8301CM Employee ClassCivil Service Add to Favorite JobsEmail this Job About the Job Non-Medical Case Management (80%): Housing Navigation- Assess new and existing clients for housing situation, stability, and access Ensure YAP client housing data is up-to-date in multiple databases Work with YAP clients as needed on a short term basis to investigate and address issues related to housing Provide direct assistance to clients to obtain or maintain housing or shelter through local community based, county, state and/or federal agencies, including: short term shelter, temporary housing programs, section 8, and coordinated entry Respond to client and social service provider inquiries to coordinate housing benefits applications and issues Establish and maintain expert knowledge of housing benefits, concerns, and resources Provide and coordinate education related to independent living skills Other duties related to the role as assigned.Employment and Finance Navigation- Assess new and existing clients employment, income, and budget Ensure YAP client income data is up-to-date in internal databases and with external programs Provide short term assistance to clients to obtain or maintain employment through local community based agencies, independent companies, and/or outside programs Establish and maintain expert knowledge of employment resourcesBenefits Review and Consultation- Provide direct assistance to clients to obtain health care benefits through county, state and federal agencies, including MNsure, Program HH (ADAP), and financial assistance programs Respond to client and social service provider inquiries to coordinate healthcare and financial benefits applications and issues Provide training and educational support for YAP clients, staff, and other service providers as necessary Establish and maintain expert knowledge of HIV and HIV-related benefits concerns and resources, including knowledge of public and private health care benefits Other duties related to the role as assigned.Program Recruitment and Referrals- Review YAP client load regularly for potential recruitment into program Consult with other YAP staff to ensure clients are receiving up to date information on the availability of housing programs Receive and respond to internal referrals from other YAP programs and staff in a timely manner Participate in agency general intake, providing referrals to appropriate YAP programs based on client need Generate and maintain a list of available housing opportunities and programs Provide appropriate and timely external referrals to client-centered best fit resourcesMonitoring, Evaluation, and Scholarship- Evaluate program for quarterly reports, writing program narratives and analyzing data for submission to grant agencies Administrative Tasks (for all above responsibilities) (5%): Program Development and Management- Data entry into state reporting documents, and internal reporting forms Participation in weekly staff consultations and supervision from the Executive Director Assist with quarterly reporting to grantors and other funders Maintain YAP Facebook, Instagram, Twitter, and other social media sites Track gift cards and financial instruments used in programs Comply with service standards outlined by HRSA Community Engagement and Health Professions Learner Education (10%): Identify and build relationships with individuals and agencies that could benefit from YAP services and knowledge (and vice-versa) Collaborate with community agencies with similar grants/goals and priority populations to promote a coordinated community response Plan and coordinate (with YAP staff and other community agencies) to present events, including: Youth Pride, Q-Quest, Queer Prom, and the annual All-Provider Meeting Professional Development and Continuing Education (5%): Attend training, such as: required MNSure Navigator certification, relevant HIV trainings and conferences, and at twelve yearly hours of additional training Maintain knowledge of (and attend training on): resources available to target populations, HIV education/counseling best practices, and motivational interviewing strategies Qualifications All required qualifications must be documented on application materialsRequired Qualifications: BA/BS or a combination of related education and experience to equal at least four years. Years of experience to include experience working with youth or people living with HIV Must pass criminal and driving record checks Must use personal vehicle for client transportation and maintain comprehensive insurancePreferred Qualifications: Ability to work flexible hours including some weekends and evenings Excellent organizational skills Ability to work with a diverse team Cultural responsiveness while working with multicultural, low-income individuals and families.
