Working Title: Intensive Care Coordinator- WraparoundProgram: Wraparound- Children's and FamilyPay Range: $26.48/hour- $29.20/hourLocation: The Dalles- This position will serve all three counties(Hood River, Wasco and Sherman County)FTE:1.0(37.5 hours a week)FSLA Status: Non- Exempt Mid-Columbia Center for Living (MCCFL) is a Certified Community Behavioral Health Clinic (CCBHC) that specializes in providing services that are both trauma-informed and recovery-oriented to persons / consumers with mental illness and addictions. Trauma-informed care includes safety, choice, collaboration, trustworthiness and empowerment for both clients and staff. MCCFL serves Hood River, Wasco and Sherman Counties and utilizes a consumer-involved, consumer-engaged and consumer-driven model. MCCFL provides a whole-person care approach to adults and children to address both physical and mental health needs. MCCFL offers a variety of programs including a mobile crisis unit, intensive case management, ongoing counseling, community support, supported employment, jail diversion, hospital discharges, hearings and more. Introduction of the Classification : The Family Care Coordinator (QMHA) is responsible for providing service planning and needs assessment to children and their families. This position assesses and monitors progress and also coordinates/refers resources as necessary. The Family Care Coordinator (QMHA) works in collaboration with community partners and the MCCFL Clinical Services Team to ensure that appropriate and effective services are provided to clients. With the high prevalence of trauma among individuals receiving mental health services, the Family Care Coordinator (QMHA) must possess demonstrated understanding of the effects of trauma on health, relationships, coping and other aspects of the lives of those we serve. General Statement of Position Responsibilities : The Wraparound Care Coordinator (QMHA) integrates the Wraparound process in accordance with the ten principles of the National Wraparound Initiative to ensure that youth and family voice are heard and incorporated in Wraparound teams. This model sees the youth as a partner & team member. The Wraparound Care Coordinator meets with youth and designated family members to identify strengths, assess needs covering all of the life domains, create mutual goals, and develop an initial wraparound plan of care. The Wraparound Care Coordinator also works directly with the family & youth to identify potential members and assemble a Wraparound Team comprised of significant individuals involved in the youth’s life, such as family members, friends, appropriate professionals, and other natural supports. Position-specific Essential Responsibilities : Intensive Care Coordination consists of organizing, facilitating, and participating in monthly team meetings with identified team members that can include youth, family, natural supports, community providers and agencies with the intention of developing a collaborative care plan with actions steps, specific responsibilities, and communication procedures. Develops, in collaboration with the youth and family members, a crisis management plan that includes action steps and identifies specific responsibilities and communication procedures. Works with youth and family members to assess strengths and needs across a number of life domains and participate in cultural discovery. Provide assistance with securing needed resources and services such as financial assistance, transportation, educational services, mental health services etc. to achieve their immediate goals and maintain independence within the community. Monitors progress on the youth’s care plan and reviews progress with the Wraparound Team on a regular basis. Track community events and coordinate attendance representation Provides linkage between the multiple systems within which youth are involved, e.g., child welfare, juvenile court, mental health, chemical dependency, special education, etc. Provide transportation for clients in agency provided vehicles when required. Essential Responsibilities of the General Classification : Assess comprehensive needs of children and families including current and potential strengths. Coordinate eligibility review team meetings and other service planning meetings for families, children, and youth. Provide assistance with securing needed resources such as financial assistance, transportation, educational services, and treatment services as appropriate. Possess knowledge of symptoms and challenges faced by children with severe emotional disturbances. Maintain familiarity with OAR’s pertaining to the mental health system. Must have the ability to effectively communicate both verbally and in writing and have the ability to work independently and complete all responsibilities in a thorough and timely fashion. Possess the ability to represent the interest of the client and the agency in a favorable light to the community. Must have the ability to work well with teams, working toward defined objectives. Provide care coordination/management services to assigned clients. Services are provided in the home and a variety of community settings with consideration to client preferences for time and location. Actively participate in joint care planning and coordination with community partner service teams and MCCFL Clinical Services Team. Facilitate the development of the individual service plan and coordinate the services outlined in the plan. Encompass families and youth as partners in the process of managing/coordinating care. Incorporate the strengths of families and youth, including the natural and social support networks on which families rely. Monitor and evaluate progress in appropriateness of services and achieving goals. Provide timely feedback/update to family regarding progress and encourage progression. Serve as an advocate for the child and family as appropriate. Utilize agency clinical software system to prepare current client records of treatment and reports as necessary and required. Responsible for the maintenance and timeliness of all records, progress notes, and other required documentation. Support the goals of continuity and coordination of care across multiple services and systems over time. Regular and reliable attendance is essential in level of care clients are anticipated to receive. Job Specifications : Incumbents in this position must be certified as a Qualified Mental Health Associate (QMHA). Any combination of experience and training equivalent to the following is qualifying: A bachelor’s degree in social work, psychology or related human services field and two (02) years’ experience in human services; OR A high school diploma and five (05) years of equivalent training and work experience; or equivalent combination of education and experience. Experience working with families and children with severe emotional disturbances is preferred. Bilingual fluency Spanish/English is preferred. Must pass all criminal history check requirements as required by ORS 181.536-181.537 and in accordance with OAR 407-007-XXXX to 407-007-XXXX. Must possess a valid Oregon or Washington Driver’s license, maintain an acceptable driving record, and be insurable for client service purposes and for travel between business offices, meetings and/or trainings. Position-specific Requirements : The Wraparound Care Coordinator must be able to work collaboratively with staff and community partners to ensure the best care for clients. This position participates in decision making processes through collaboration with clinical services supervisors, managers, psychiatrists, other care coordinator(s), and staff members for emergency and stabilization of psychiatric patients, including emergency situations. The work assigned is diverse and involves addressing new and unusual circumstances in which outcomes may negatively affect costs, employee morale or clients. The work regularly involves a degree of unpredictability and disruption of planned tasks requiring a flexible time management approach. The Wraparound Care Coordinator will be tasked with managing the mental demands of the position while monitoring and supporting client care and staff relationships. Other Required Knowledge, Skills and Abilities : Knowledge : Knowledge of symptoms and challenges faced by children with severe emotional disturbances; knowledge of assessment and referral processes and procedures; knowledge of medication management; knowledge of MCCFL-based and other community-based services; knowledge of mental health and medical assessment, treatment, and service terminology; knowledge of treatment/service planning; knowledge of medical records maintenance. Skill : Skill in the use of clinical software systems; skill in the use of standard office software; skill in keyboarding. Ability : Ability to understand and effectively apply the concepts and practices of a trauma-informed philosophy in performing the work. Ability to work within strong professional ethics and rules of confidentiality to provide effective services to clients. Ability to effectively work with clients to encourage and facilitate engagement in the treatment process; ability to maintain a strong service orientation; ability to read and understand a service plan and to effectively coordinate identified services; ability to write and maintain timely and cogent notes, reports and other professional documents; ability to organize and execute work effectively under the pressure of time-sensitive deadlines; ability to work with grace and good humor under pressure and to meet multiple deadlines; ability to work cooperatively with other departments and service providers; ability to communicate effectively, both orally and in writing, and within trauma-informed care guidelines; ability to effectively represent client and Agency interests to community partners and other stakeholders; ability to establish and maintain harmonious work relationships with co-workers at all levels of the organization, consultants, contractors, community partners, and consumers. Leadership Received : Intensive Care Coordinators work under the supervisory leadership of a Clinical Supervisor or Program Manager. The designated supervision emphasizes independence of method and process while indicating accountability for results. Leadership Exercised : This position is not responsible for providing supervisory leadership to any other classification. Physical Demands : The Intensive Care Coordinator must have the ability to quickly assess and respond to concerns related to interpersonal and environmental safety as well as client care. While performing the essential duties of this position, the employee is regularly required to use office automation including computer and phone system that requires fine manipulation, grasping, typing, and reaching. The employee may be required to sit or stand for several hours at a time, depending on environment and need. This position may occasionally be required to lift up to twenty (20) pounds, and to kneel, reach, bend or squat while in the office or in the community. Work Environment : Work is performed in various settings, including office environment, schools, community settings and other agency stakeholder facilities. The office environment is typically with moderate stimuli and may often result in exposure to increased sensory experiences. Community settings, such as residence, schools, police precincts, respite facilities, hospitals and other public spaces may pose additional risks or discomforts which require safety precautions. Intensive Care Coordinator is expected to perform work responsibilities in the same settings in which employees are required to work.
