Job Title: SOS Case Manager Summary of Job Duties : The Case Manager is responsible for assessing, coordinating and providing services for women who are admitted to the program. Qualifications : To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. This position requires a HS diploma and 2 years job related experience. Bachelor degree in related field is preferred. Knowledge of women’s issues in recovery is helpful. If in recovery, a minimum of 2 years continuous abstinence from drugs/alcohol required. Submit to and pass a criminal record check and a drug screening test. Hours : Monday through Friday 8:00 am until 4:00 pm. Evening and weekend hours may be required. This position reports directly to the Cumberland River RHOAR Program Director. Essential Duties and Responsibilities: Include the following: In coordination with the Program Director, plans, coordinates, and evaluates all programmatic and administrative needs and responsibilities of clients. Assess client employment and job training needs. Teach open classes to address job/interview skills. Follow up with clients and employers to ensure successful job placement. Prepare and maintain a list if agencies and contact information for referrals. Maintain and submit food stamp applications for clients. Coordinate with parties involving the food stamp cards of clients who have left the program. Attend Community. Coordinate with courts, judges and referral agencies to provide services. Works regionally with police, medical personnel, and other community. Service agencies and individuals to provide and promote optimal services to clients. Screens all clients for admission and maintains a waiting list. Provides daily supervision to ensure accurate, complete, and up-to-date records are kept and all clientele are receiving appropriate care. May be required to perform drug screenings on clients as needed. Facilitates communication with other Recovery Kentucky Program staff. Attends and participates in all required meetings. Demonstrate the ability to work effectively with other team members. Other duties as designated by Program Director. Required Job Skills : Oral and Written Communication Skills Interpersonal Skills Ability to exercise good judgment, tact, diplomacy and compassion when problem solving, handling conflict or in a crisis situation Ability to complete record keeping in a timely fashion. Ability to get along with diverse personalities and maintain effective working relationships. Ability to work well with people of all races, backgrounds and needs. Ability to make decisions quickly based on available information and take action. Attentive to all aspects of the work environment. Knowledgeable of Behavioral Health Concepts and how to incorporate concepts into practical application. Ability to work as part of a team and to positively represent the agency in the community. Knowledgeable of the stages in recovery for behavioral health and/or chemical dependency. Physically able to reach, push, pull, stoop, stand, walk and lift up to 20 pounds. Possess a valid Driver’s License, maintain a safe driving record, and maintain the required minimum vehicle insurance amount for the State of Kentucky, if required for job performance. Maintain CPR/First Aid licenses. Work Environment: The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job. While performing the duties of this job, the employee occasionally works in outside weather conditions or may be off-site from normal office location. The noise level in the work environment is moderate to loud. Environmental Data and Job Hazards : Hours of work are structured 8.0 hour blocks of time but may be subject to irregular and/or flexible hours based on needs or demands Position may require duties to include routine or reasonable anticipated tasks or procedures where there is a degree of actual or potential exposure to blood or other infectious materials. All staff are required to participate in yearly trainings regarding infection control and safety in the environment including hazardous waste and blood borne pathogens. All new staff must attend training regarding handling physical, verbal threats, acts of violence or other escalating and potential dangerous situations including when police need to be summoned. All staff must be certified in Cardiopulmonary Resuscitation within one month of employment and maintain the certification, if providing direct care. Powered by JazzHR
