About the Opportunity The Diabetes Management Centres (DMCs) throughout Nova Scotia Health fall under the Primary Health Care portfolio. The DMCs work with primary care providers to provide patients with the support they need to manage their diabetes. The goal of the DMCs are to provide education and self-management support for patients to manage their blood sugars, cholesterol levels, and blood pressure.
