Job Summary: Under the direction of the Community Engagement Program Manager, this role demands excellent people skills and a passion for creating relationships. All interactions will be person centered, be racially and culturally competent, strengths-based, trauma informed, and have a harm-reduction perspective. This position provides group and individual services, including service navigation support and living skills training. This Community Engagement Specialist position will have a strong focus on medical and psychiatric care coordination for residents and will support partners that deliver medical and psychiatric care on site. Responsibilities: Acts as a liaison with on-site Community Medical providers and Esperanza staff. Responsible for the scheduling of initial evaluations and follow-up appointments. Enhance continuity of care and adherence to follow up plans of care by: Obtain releases of information with medical providers and upload relevant medical records into Apricot, as needed and consented by the client. Communicate with Case Management through case notes, regarding known follow up steps needed to adhere to treatment recommendations. Provides medication monitoring and education specific to each client’s needs to increase appointment attendance, adherence to treatment recommendations and build skills to manage symptoms. Support community members and navigate access to requested programs, resources and services, both internally and externally of the organization. Engage in a daily practice of building community and model neighborly behavior, through outreach efforts, facilitating groups, recreational activities and social skill building opportunities. Facilitate communication to engage clients and keep them informed of how to get the most benefit out of TOOF services. De-escalation and crisis intervention as needed. Document services provided in HMIS, Apricot or other-directed documentation methods. Attend and participate in team meetings and performing other duties as assigned. Qualifications & Skills: Certification as a Medical Assistant Experience working with individuals who are homeless and/or at risk of homelessness. Ability to promote a culture that encourages low barrier access to services, with a culturally and ethnically diverse population. Ability to maintain professional boundaries, engagement skills, de-escalation and conflict media Excellent written and verbal communication skills Well-organized, able to multi-task and complete assignments in an effective manner. Competent technology skills, including experience with Microsoft Office (Word, Excel, etc.). Ability to work collaboratively with co-workers and external partners. Apt in problem-solving, and motivating others. High professional standards and dedication to the organization’s mission, vision & values. Proficient in Harm Reduction principles and practices Valid Driver's License Weightlifting requirement of 35lbs Schedule. Schedule: Must be willing to work weekends and will be on a regular and predictable schedule of 10-hour shifts. This position will be Sunday through Wednesday schedule to coincide with on-site health care partner schedules. Benefits and Salary: Salary $56,645 Dental insurance Offered.
