EDUCATION & EXPERIENCE MINIMUM EDUCATION & EXPERIENCE Graduate of a licensed and accredited school of practical nursing Florida LPN License (Active) 1 year of psychiatric experience 1 year of experience in health education PREFERRED EDUCATION & EXPERIENCE 3 years of psychiatric experience KNOWLEDGE & SKILLS Must be knowledgeable of and have the ability to interpret the Backer Act, Marchman Act, confidentiality, policies, procedures, client rights, and abuse reporting regulations Self-starter: require minimal supervision regarding time management and prioritizing work assignments Proven track record of having the capacity to define problems, collect data, establish facts, and draw valid conclusions Capability to administer injections, perform lab duties (draw blood & collect samples), dress wounds, etc.; Infection Control Familiarity with medications, side effects; risk of addiction, bloodborne pathogens, OSHA Standards Must be knowledgeable on the needs of the chronically, mentally ill Ability to record charts and forms in a complete, accurate, and timely manner Excellent communication skills, both verbal & written, with professionals, staff, & client/families AGE SPECIFIC COMPETENCIES ADULT Knowledge of medications/dosages Assess changes inproductive status (e.g. economic, physical) as a result of illness Assess and address life stages concerns of clients GERIATRIC Medication changes ; knowledge of medication interactions Assess nutritional and dependency needs Monitor intake/elimination patterns ; assess audio/visual problems RESPONSIBILITIES Documents services in accordance with Center policies and procedures and regulatory agencies including but not limited to, accuracy reviews, writing progress notes, activities or significant behaviors, record medications and side effects Document on a Medication Administration Record(MARS) which includes the following: name of client, name of medication, dosage of medication, route given (in BTH, all injectable medication would be administered by a RN), time of administration, initial of staff supervising or administering medication, record number of client, drug allergies and initial and complete signature of staff supervising medication Submits service activity logs, weekly and monthly schedules, timesheets and mileage to Supervisor Complete 12 hours of staff development and training per year Supervise PPD program for client; and staff in residential programs Maintain and reconcile medication inventory for all clients within the Residential Program. Ensure that medications are ordered via SCMHC pharmacy and/or community pharmacies in a timely manner and according to client’s refill schedule. Act as Liaison between Residential Program and other Providers, such as, Primary Care Physicians, Specialists, Center Med Clinic/ Psychiatrist, Case Managers, and other client care providers. Coordinate and track client lab orders. Monitor for medication, side effects, vital signs, immunizations, TB testing, etc. Supervise and administer medication, orally and by injection , as needed Ensure appropriate routine and/ or critical medical follow-up Provide special instructions to resident and home staff related to medical diagnosis(allergies, medication, diet, special emergency plans related to ongoing medical conditions) Coordinate care with clinics, psychiatrists, therapists, and hospitals Monitor BTH: sick call (individual sheets and monthly logs), problem list (physical, dental and mental health), medical classification and health discharge summary Educate clients and staff on medication, side effects, preventative medicine/wellness program and nutrition Lead hygiene, HIV/AIDS, nutrition, medication education , etc. Mental Health/Behavioral Teaching , specifically regarding proper Medication Assistance protocols Health Teaching included but not limited to, ADL’s, seizures, diabetes, etc. Review and coordinate residential manuals in relation to infection control, OSHA, medication management, and Center policies and procedures Performs all other duties as assigned Powered by JazzHR
