A rehabilitation network in California is actively seeking a licensed Physical Therapist (PT) to join their growing staff at one of their facilities in the Greater San Diego area. In this role, the Physical Therapist (PT) will be responsible for assisting patients to reach their maximum performance and level of functioning while learning to live within the limits of their capabilities.About the Opportunity * Schedule: Monday to Friday * Hours: 8:30am to 4:30pm * Patient Population: Adults * Patient volume: 8 patients per day * EMR: PinPoint and UDSResponsibilities:The Physical Therapist (PT) will: * Assess patients' needs * Observe and evaluate treatment affect, accomplishment of short- and long-term goals * Perform an appropriate assessment on each patient and performs reassessments, as per policy * Formulate and update patient specific plan of care for physical therapy * Report patient progress and barriers to discharge in each patient team conference * Formulate a teaching plan based on identified learning needs and evaluate effectiveness of learning; family is included in teaching, as appropriate * Manage and operate equipment safely and correctly * Interact professionally with patient/family and involves patient/family in the formation of the plan of care * Perform other duties, as needed:Qualifications: * CA Physical Therapist (PT) license * Current BLS/CPR certification * Great interpersonal skills * Excellent communication skills * Highly organizedDesired Skills: * Inpatient Rehab experience
