Company: Goodwill Full-time Compensation: $45,000 + Benefits General Purpose Coordinate Prevocational and/ or Day Habilitation services for a case load of up to 30 of the program participants with Intellectual and Developmental Disabilities. Engage each program participant in a Person-Centered Planning process to identify their goals and implement services and activities to help them achieve their highest level of independence. Coordinate group & individual scheduling of activities and services, regularly visit program and volunteer sites, oversee Direct Support Professionals (DSPs) implementation of services to program participants, write / update program participant’s Staff Action and Person-Centered Plans. Essential Functions Complete required documentation (including Staff Action Plans, Monthly Case Notes, Person Centered Plans, New Start Packets, Incident Reports, and other required documentation) efficiently and to funding source and agency standards within specified timeframes. Ensure program participants caseload meet monthly attendance goals. Prepare monthly attendance report for Program Manager/Day Services Supervisor. Engage each program participant in Person Centered Planning. Use their Person-Centered Plan and feedback as the basis for developing Staff Action Plans, Goals, Staff Supports & Services, and in group & individual monthly schedule development. Each program participant’s goals, interests, needs, and skills must be taken into account when developing schedules. Solicit feedback, observations, insights, & recommendations from the program participants, DSPs, & Job Developers regularly on program participant’s progress & changes to make to schedules & Staff Action Plans. Responsible for submitting & balancing Petty Cash reports, tracking Metro Card distribution & reconciliation, submitting monthly transportation reimbursements, and conducting monthly fire drills at offsite locations or supervising Senior DSP if one is assigned. Visit hub sites & community sites to provide services to the program participants, supervise activities, evaluate program participant’s progress, and monitor the quality of services/ activities and interactions between DSPs and program participants. Responsible for ensuring DSPs provide daily services as written in the program participant’s Habilitation Plan and accurately enter time & attendance and services provided to the people that we support on a daily basis. In accordance with OPWDD and agency requirements modifies his / her own communication to ensure understanding & respect when interacting with program participants; including modifying tone of voice and use of verbal & nonverbal techniques to meet the individual’s communication needs. Use electronic health records, MS word and Excel, and online applications to complete work. Exercises good judgment and discretion in all communications and when making decisions that impact program participants, the group of program participants, and / or in crisis situations. Demonstrate the following desirable professional qualities: Professional demeanor, excellent punctuality & attendance, reliability, readily adapts to changes in work assignments, and good communication skills. For Prevocational Services, develop & support the program participant’s skills to help them meet the productivity expectations of the work place. Develop and maintain positive & productive relationships with coworkers, supervisors, and other colleagues, program participants, parents & caretakers, support teams, and internship / volunteer sites. Understand, identify and address staff and team development needs by working with Staff Trainer. Other duties as needed or required. Qualifications Education: (1) Two years of work experience in OPWDD funded services, or other social service programs, as a Direct Support Professional or equivalent + High school diploma or equivalent required OR (2) A bachelor degree from an accredited school in related field. One year of work experience in mental health, vocational rehabilitation, or case management preferred. Skills: Strong interpersonal & communication skills, strong organizational and time-management skills, ability to handle multiple tasks and prioritize demands, strong written communication, ability to work independently with minimal supervision and approach problems in a solution-oriented manner. Frequent travel & field work within the boroughs required. Attendance required at Individual’s semiannual meetings that may occur evenings and / or at off site locations.
