The center provides services in a culturally sensitive manner, offering translation services when needed, and ensures that no one is denied care due to an inability to pay. As a Federally Qualified Health Center, LCHC accepts all insurances, helping patients navigate their coverage. Role Overview The Community Health Worker (CHW) at LCHC is integral to our mission of providing exceptional health care services. In this role, the CHW assists our Providers and clinical staff in delivering quality health care. The CHW is pivotal in guiding patients and their families through the maze of community services and resources, fostering the adoption of healthy behaviors. This position involves a blend of care management and community outreach, functioning in support of our providers through an integrated approach. A key aspect of this role is to promote, maintain, and enhance the health of our patients and their families, including providing social support, informal counseling, and advocacy for individual and community health needs. The CHW also conducts health screenings and home visits, playing a vital role in our patient-centered care model. Qualifications Demonstrated experience in community health work or a related field. Strong interpersonal skills with a proven ability to build rapport with patients and families. Knowledge of community resources and services. Ability to work effectively in a team-oriented healthcare environment. Strong organizational and communication skills. Capacity for providing informal counseling and social support. Familiarity with health screening procedures is advantageous. Requirements Commitment to full-time employment. Flexibility to perform home visits and community outreach activities. Adherence to LCHC's policies and ethical standards. Willingness to undertake continuous training and professional development. Ability to advocate effectively for patients and community health needs. Capacity to work collaboratively with different departments within LCHC.
